今回はワンちゃんのお腹が大きくなっているときに考えられることについてまとめてみました。

In this issue, I have compiled a list of things to consider when your dog’s belly is getting bigger.

包括的な内容になるので、個別の疾患については別でまとめたいと思います。

Since this will be comprehensive, I will summarize individual diseases separately.

緊急性の少ないものから順に書いていきますね。

I will write in order of least urgent first.

そもそも太っている場合 If your dog are fat to begin with

まずはそもそも太っている場合です。

The first is if you are fat to begin with.

この場合は元気・食欲には問題が無く、摂取カロリーが消費カロリーを上回っているのが問題です。

In this case, there is no problem with energy and appetite; the problem is that calorie intake exceeds calorie consumption.

動物の体格の指標としてはボディ・コンディション・スコアというものが用いられますが、胸部を触ったときに軽く肋骨が触れて、腰のくびれが認められる状態が理想だと考えられます。

Body condition score is a indicators of animal body size, and the ideal condition is one in which the ribs lightly touch when the chest is touched and narrow part of the waist is recognized.

摂取カロリーの見直しとしては口に入る全ての物を把握し、フードやおやつをどれぐらい与えているか、人の食べ物を与えていないかなどについてチェックします。

To review calorie intake, we take stock of everything that goes into the mouth and check how much food and treats are being fed and whether or not human food is being fed.

摂取カロリーを減らしていく方法としてはまずおやつを減らすことをお勧めします。それから人の食べ物を与えてしまう場合はできる限りこれを止めましょう。

The first way to reduce calorie intake is to cut back on treats. Then, if you give them human food, stop this as much as possible.

フードはなるべくカロリーを抑えた上でバランスのとれたものを選択するのがお勧めです。

It is recommended that food choices be as balanced as possible with as few calories as possible.

消費カロリーに影響しそうな病態もチェックします。甲状腺機能低下症などです。

We also check for medical conditions that may affect calorie consumption. These include hypothyroidism.

心臓が悪い、関節を痛めているなどの異常が無い場合は、散歩などで消費カロリーを増やすのもお勧めです。

If there are no abnormalities such as heart problems or injured joints, it is recommended to increase calorie consumption by taking a walk.

妊娠している場合 In case of your dog is pregnant

もちろん避妊手術をしておらず、他のワンちゃんと交尾してしまった場合は妊娠の可能性もあります。

Of course, if the dog is not sterilized and has mated with another dog, there is a possibility of pregnancy.

ワンちゃんの妊娠期間は約2ヶ月なので、特に妊娠後半は急速にお腹が大きくなってきます。

Since the gestation period of a dog is about two months, the belly grows rapidly, especially in the second half of pregnancy.

もし妊娠を望まない場合は、女性の病気を予防するためにも早め避妊手術（生後6ヶ月以降）を受けることをお勧めいたします。

If you do not wish your dog to become pregnant, I recommend that your dog undergo early sterilization(>6 months of age) to prevent female diseases.

腹筋が衰え、脂肪がつきやすくなる病態 Pathology that causes abdominal muscles to weaken and fat to accumulate.

これはずばり副腎皮質機能亢進症です。

This is plainly hyperadrenocorticism.

副腎皮質の機能が亢進すると、ステロイドが多く分泌されることによって全身の筋肉が弱くなり、脂肪がつきやすくもなって結果的に腹囲膨満が生じます。

When the adrenal cortex is hyperactive, the production of high levels of steroids weakens the muscles of the body and also makes it easier to gain fat, resulting in abdominal bloating.

この病態はすぐに急変する可能性は低いですが、血栓症を引き起こしたり、感染への防御が弱くなったり、傷が治りにくいなどの問題を引き起こす可能性があるので放置せずに治療することが大切です。

Although this condition is unlikely to change quickly and suddenly, it is important to treat it without neglecting it because it can cause thrombosis, weaken defenses against infection, and cause problems such as wounds that do not heal well.

腹部臓器の腫大が認められる場合 If the abdominal organs are enlarged

腹部臓器（肝臓、脾臓、腎臓、副腎、胃腸、膀胱、子宮、前立腺など）の腫大が認められる場合は良性や悪性の腫瘍、その他には子宮への液体貯留などが考えられます。

If the abdominal organs(liver, spleen, kidneys, adrenal glands, stomach, bladder, uterus, prostate, etc.) are enlarged, there may be benign or malignant tumors or fluid retention in the uterus.

いずれの病態であっても、手術が根本的な治療になる可能性が高いと考えられます。

In either condition, surgery is likely to be the fundamental treatment.

もちろん、腫瘍であれば腫瘍血管との関連性や癒着の有無などによって治療の難易度は変化するでしょうし、手術自体が不可能な場合もあるでしょう。子宮の水腫や蓄膿症でも高齢での罹患の可能性が高く、合併症も多いため、予後が難しい場合も多いです。

Of course, if it is a tumor, the degree of difficulty of treatment will vary depending on its association with tumor vessels and the presence or absence of adhesions, and in some cases the surgery itself may not be possible. Hydrocele and pyometra of the uterus also have a difficult prognosis in many cases because of the high likelihood of the disease at an advanced age and the many complications that may arise.

腹腔に液体が貯留している場合 Fluid retention in the abdominal cavity

腹腔に液体が貯留している場合、腹水の場合もありますし、出血している場合も想定されます。

If fluid is accumulating in the abdominal cavity, it may be ascites, or it may be assumed that the patient is bleeding.

腹水の場合、腫瘍や炎症などによって液体貯留する可能性や、心臓が悪く循環が上手くいかないために液体貯留する場合があるでしょう。また低タンパク血症になることで腹水が貯留することも考えられます。

Ascites may be caused by fluid accumulation due to tumor or inflammation, or it may be caused by poor circulation due to a cardiac disease. Hypoproteinemia may also cause ascites.

出血している場合は、外傷性だったり、腹腔臓器の腫瘍が破裂したりして出血が起こると血腹となります。

If bleeding is traumatic or caused by a ruptured tumor in an abdominal organ, it is a hemoperitoneum.

腹水であっても血腹であっても、その根底には重篤な疾患が存在する可能性が高いです。

Whether it is ascites or hemoperitoneum, there is likely to be a serious underlying disease.
