お尻周りが膨れていても、元気・食欲があり、便も尿も普通に出ている場合は慌てなくても良いかもしれません。

会陰ヘルニアとは？ What is perineal hernia？

会陰とは肛門と陰部の間の部分を言います。

The perineum is the area between the anus and the genital area.

会陰ヘルニアとは、会陰部に穴が開いて、腹腔の臓器などがそこから脱出している状態のことを言います。

A perineal hernia is a hole in the perineum from which organs and other parts of the abdominal cavity have prolapsed.

穴が開くと言っても、本当に穴が開いているわけではなく、お尻を構成している筋肉群の隙間が空いてしまってそこが穴になるという状態です。

A hole is not really a hole, but rather a gap in the muscle group that makes up the buttocks, and that is where the hole is.

原因としては男性ホルモンの影響、会陰部の筋肉萎縮が生じる原因(断尾など)、加齢による影響、過度のしぶりが考えられています。

Possible causes include the effects of male hormones, causes of perineal muscle atrophy (e.g., tail transection), the effects of aging, and excessive squeezing.

検査 Examination

会陰ヘルニアの検査としては、直腸検査、レントゲン検査、ヘルニア部のエコー検査が含まれます。

Examination of the perineal hernia includes rectal examination, radiographic examination, and echocardiography of the hernia area.

また、治療は基本的には手術になるため、全身麻酔をかけることができるかどうかの検査も重要です。全体的な血液検査や胸部レントゲン検査を含みます。

It is also important to test for the ability to administer general anesthesia, since treatment is essentially a surgical procedure. This includes overall blood tests and chest x-rays.

もし全身麻酔をかけることができないような全身状態であれば、手術を行う前に点滴治療などの内科治療を実施します。

If the patient is in such a general condition that general anesthesia cannot be administered, medical treatment such as intravenous infusion therapy will be administered before surgery is performed.

治療 Treatment

治療は基本的には手術で実施されます。

Treatment is basically carried out by surgery.

手術によって隙間の空いた筋肉や靱帯を縫い合わせ、穴を塞ぎます。

Surgically, the gaping muscles and ligaments are stitched together to close the hole.

また、あまりにも筋肉が萎縮している場合は、メッシュなどの人工物で穴を塞ぐ場合もあります。

また、”会陰ヘルニアとは？”の部分でご説明したように、この疾患は男性ホルモンの影響が大きいと言われているので、去勢手術も同時に実施します。

加えて、結腸や膀胱が陰部へ向かわないように、腹壁へ固定する場合もあります。

この疾患は高齢のワンちゃんで生じることが多いため、飼い主さんと獣医さんの話し合いの中で手術は避けたいということになるかもしれません。

その場合の対策は、排便困難に対する軟便化処置、摘便を中心とした内科療法であり、もし膀胱がヘルニア孔に入ってしまう場合は、内科的な管理だけでは対処が困難かもしれません。

In such cases, the measures are medical therapy, mainly softening procedures for defecation difficulties and stool removal; if the bladder enters the hernia hole, it may be difficult to cope with medical management alone.

また、摘便を繰り返す中で直腸が破綻し、便が骨盤腔へ漏れてしまい、破滅的な状況になる可能性もあります。

• 全身麻酔：8000～1万円ぐらい
• 手術：片側10万～15万円ぐらい、両側：20万～25万円ぐらい
• 入院費：一日あたり1万～1万5千円ぐらい
• トータル：1週間程度の入院で、30万～40万円ぐらい
